The only way that can happen is when the semi-automtic system is badly modded to create a full auto, but than the entire mag would be emptied.
Pistols use the propellant to prepare the mechanism for firing another round. In theory the nipple catching that mechanism before being released again by using the trigger could fail, but than the shot is fired immediately.
A good Pistol or bad Pistol is always relative to the shooter. In most areas off the U.S.A. a gun is good, because the alternative is no gun.
